employment, for a reasonable monthly contribution. We offer Preferred Provider Option
(PPO) plan. |

Vision Coverage
|
|

In any 12-month period, the plan reimburses you for the full cost of an eye exam and
partial reimbursement for eyeglasses,
contact lenses, or frames. |

Prescription Drug Coverage

The plan includes a prescription drug card
that entitles you to prescription drugs for
a co-payment. For long-term maintenance
drugs such as blood-pressure medications,
there is a convenient mail order plan. |

Dental Insurance

| The
McNICHOLS
dental plan pays 100% of charges for preventive services such as cleaning and checkups, plus 80% (after $100 deductible)
for general services such as fillings and extractions, and 50% for bridges and crowns. There is a $1,500 annual maximum on dental claims. Children’s orthodontics charges are covered 50% up to a $1,500 maximum. |

Medical Flexible Spending Account with Debit Card
|
|
|
| The Flexible Spending Account
provides you a "tax break" on out of pocket medical costs under IRS Section 125. You agree that a portion of your pre-tax salary will be set aside in an account. This amount is taken out of your pay over 24 paychecks during the year. Your FSA Debit Card works similar to a saving account, although money is allocated for specific uses. |

Group Life Insurance
|
|
|
| You receive life insurance, paid for by
McNICHOLS, in an amount equal to your annual pay, to a maximum benefit of $50,000. Double the amount is paid to your beneficiary in case of your accidental death. |

Optional Supplemental Life Insurance
|
|
|
| Optional supplemental life insurance is available to you and your
dependants at reasonable group rates depending on
your age and amount of coverage desired. |

401(k) and Employer Match, Profit Sharing
|
|

| To provide funds for retirement,
McNICHOLS
(after certain length-of-service requirements) may contribute to your individual Profit Sharing account an amount equaling a percent of your earnings, as determined by the Board of Directors.
Also, a 401(k) savings plan allows you to save any percentage of your earnings, with the Company contributing 50% for every dollar saved up to 6% of your pay. Profit Sharing and 401(k) funds plus investment gains accumulate on a tax-deferred
basis, and both plans are subject to
change. |

Bonus Potential

| Although not a guarantee, the Company endeavors to pay
incentives or bonuses to all employees each year.
Incentives and bonuses are determined by overall evaluation of your work performance and are always subject to the profitability of the company. |

Paid Time Off (PTO)
|
|

Full-time employees receive a prorated bank of days in the calendar year they start and 17 days in their second year of employment. The year you celebrate your fifth anniversary you receive 22 days. In your 15th anniversary year you will receive 27 days. Acceptable uses of PTO include, but are not limited to:
· Vacation
· Illness for self, spouse or child
· Any personal time
· Observation of holiday not included in the
company paid holiday schedule
· Unexpected emergencies
· Time off with family member for medical testing or surgery

Tuition reimbursement is
offered to all full time (defined as 30 or more hours per week) associates with at least one calendar year or 1,250 hours of service. Associates in good standing become eligible for reimbursement during the first semester following the completion of one year of service. |
